【系務公告】114-2提醒落實期中學習評量監考責任與遵守考試規則 114-2 Midterm Assessment Proctoring and Examination Regulations Announcement

親愛的老師您好:

本學期期中學習評量將於第9週前後(約4月20日至24日)由任課教師自行安排進行。為維護考試公平性與秩序,請各位老師務必落實監考責任,並協助學生遵守考試規則。重點如下:

📢 考試規則重點提醒:

1. 僅可攜帶授權之資料、用品及計算機。

2. 禁止攜帶通訊設備、記憶型電子產品或干擾考場之物品。

3. 不可於身上、文具、桌面等處書寫提示訊息。

4. 嚴禁夾帶、傳遞、調位、頂替、代考或攜卷離場。

5. 不得窺視、交談、或讓他人觀看試卷。

6. 考試期間須聽從教師指示,不得擅自離場或破壞秩序。

7. 試卷與答案卡不得交換或代填個人資料。

8. 不得竊題或將試題洩漏給其他同學。

📢 違規處理流程:

1. 蒐集並保留違規證據(如拍照)。

2. 填寫《台北校區違規紀錄表》或《桃園校區違規紀錄表》。試場內有任何情況發生需詳細紀錄,並清楚敘明人、事、地、物、時及處理狀況,以作為後續扣分或獎懲會議參考之依據。

3. 輔導學生填寫《學生報告書》。

4. 連同佐證資料繳交至台北課務組(王淑雯老師)或桃園教務組(彭鈺娟老師)。

5. 由教務處會簽學務處,必要時召開獎懲會議。

Dear Teachers:

The midterm assessments for this semester will be arranged by individual course instructors around Week 9 (approximately April 20 to 24). To ensure fairness and order during the exams, we kindly ask all teachers to fulfill their proctoring duties and assist students in adhering to the examination rules. Key points are as follows:

📢  Important Reminders on Exam Regulations:

1. Only authorized materials, supplies, and calculators may be brought into the exam room.

2. Communication devices, memory-based electronic products, or any items that may disrupt the exam environment are strictly prohibited.

3. Writing hints or messages on the body, stationery, or desk surfaces is not allowed.

4. Cheating methods such as smuggling notes, passing information, changing seats, impersonation, or taking exam papers out of the room are strictly forbidden.

5. Students must not peek at others’ papers, converse, or allow others to view their exam paper.

6. Students must follow the teacher’s instructions during the exam and may not leave the room or disrupt order without permission.

7. Answer sheets and test papers must not be exchanged, and students must not fill in personal information on behalf of others.

8. Stealing or leaking exam questions to others is prohibited.

📢  Violation Handling Procedure:

1. Collect and preserve evidence of the violation (e.g., photos).

2. Complete the “Taipei Campus Violation Report Form” or “Taoyuan Campus Violation Report Form”:If any incident occurs in the examination room, it must be recorded in detail, clearly stating the people involved, the event, location, objects, time, and how it was handled. This record will serve as a reference for subsequent score deductions or disciplinary/reward meetings.

3. Advise the student in completing the “Student Statement Form.”

4. Submit the documents along with any supporting evidence to: Curriculum Affairs Section on Taipei Campus (Ms. Wang Shu-Wen) or Academic Affairs Section on Taoyuan Campus (Ms. Peng Yu-Chuan).

5. The Division of Academic Affairs will review the case with the Division of Student Affairs and, if necessary, convene a meeting of the Student Awards and Punishment Committee.
