| 適用月份   :◎5月份 Applicable Months: ◎May適用對象:◎大學部 Intended Audience: ◎Undergraduate 【學務處重要通知】學生請假注意事項 1. 依據本校學生請假規則第一條內容所述:學生請假(除一日之事假、例假、身心調適假【前二日】)應至「電子公文與表單系統」上傳請假證明文件。缺課日起算限七日內辦理完畢,逾時者不予補辦。 2. 請病假首日(第1天)須有請假證明文件為憑。例如:就醫證明為1月4日,不能請1月3日病假。【如有人為不可抗拒因素,導致請病假首日無法就醫,請備妥相關證明文件至學務處生輔組/桃園行政處學務組洽詢。】 3. 本學期畢業班課程結束時間為114年5月23日,因操行成績結算時間在即,請本學期僅修讀畢業班課程同學,務必於114年5月29日以前完成請假作業,逾時不予辦理。 4. 本學期非畢業班課程結束時間為114年6月20日,請同學務必於114年6月26日以前完成請假作業,逾時不予辦理。 
 
 【Important Notice from the Student Affairs Division】Guidelines for Student Leave Application 1. According to Article 1 of Regulations for Requesting Leave for Students: Except for one-day personal leaves, menstrual leaves or mental health leave (within the first two days), all other leave requests require upload of supporting documents through the ” E-Requests and E-Form System”. Leave application should be made within 7 days after the day of absence. Over-due applications will not be taken. 2. A medical certificate or supporting document is required starting from the first day of sick leave. For example: If the medical certificate is dated January 4, sick leave cannot be filed for January 3. [Note: If unforeseen circumstances prevent medical consultation on the first day of sick leave, please prepare relevant documentation and contact the Student Advising Section of the Student Affairs Division or the Student Affairs Section of the Taoyuan Campus Administration Division.] 3. The semester’s graduating courses will end on May 23, 2025. As conduct grades will soon be finalized, students who are took only in graduating courses this semester must complete all leave applications by May 29, 2025. Over-due applications will not be taken. 4. The semester’s non-graduating courses will end on June 20, 2025. Students must complete all leave applications by June 26, 2025. Over-due applications will not be taken.
